Thrive in Wellness is a private-pay practice and does not bill insurance directly for evaluations. A superbill may be available for eligible clinical services; however, any reimbursement depends on your plan’s out-of-network benefits and is not guaranteed. Please confirm your coverage with your insurance provider before scheduling.
Insurance carriers rarely, if ever, reimburse for evaluations requested for workplace leave, disability claims, or other administrative purposes.

Evaluation fees vary by service type. Starting fees are:
Adult ADHD evaluations: $1,200
Veteran disability evaluations and VA nexus letters: $1,600
FMLA (Intermittent or continuous): $400
Short-term disability evaluations: $1,500
Long-term disability evaluations: $2,000
We require a 50% deposit at the time of booking, with the remaining balance due on or before your scheduled appointment.
We accept all major credit cards and HSA/FSA cards.

Step 1: Schedule a Consultation
A 15-minute phone call to discuss your needs, ensure we are the right fit, and provide an exact cost quote.
Step 2: Record Review
External records (like school or workplace records or c-files) are reviewed.
Step 3: Clinical Interview & Testing
We gather your background history and administer standardized testing batteries. This can take anywhere from 2 to 6+ hours depending on the complexity of the evaluation.
Step 4: Analysis & Report Writing
The assessments are scored, reviewed, and interpreted. A comprehensive diagnostic report is written with treatment recommendations and any necessary additional forms.
Step 5: Final Report Delivery & Feedback (Optional)
We meet to review the results, explain the diagnostic findings, answer any questions, and discuss recommendations or next steps.

Typically, final reports are completed 2 to 3 weeks after your appointment.

Yes. For appropriate situations, we can complete:
