How the Process Works
Step 1:
Book your Appointment
Schedule online for $1,000. HSA/FSA are accepted. Secure intake forms are sent immediately after booking and must be completed prior to your session.
Step 2:
The Clinical Interview
Plan for about 1 hour via secure telehealth. We will complete a clinical interview focused on your condition, your actual day-to-day job duties, and how your current work environment impacts your functional limitations.
Step 3:
Analysis
We’ll take it from here. We review your clinical information, job duties, and functional limitations to create clear, well-supported documentation that connects your needs to specific accommodations.
Step 4:
Written Report
A signed clinical report detailing your functional limitations and the accommodations that may address them, delivered within 5 business days. Submit it to HR as part of your accommodation request.
